110-300-0165(2)(a) Licensee had a small play toy ball in the outdoor play area that was less then one and three-quarter inches or less that posed a risk of choking while on this site visit. 110-300-0165(2)(c) licensee had a long rope accessible to the children in the backyard licensed space by the gate that could create a loop, licensee had a cord to a speaker on the kitchen sink accessible, a cord from a bullet kitchen aid in the kitchen on the counter that was accessible to the children, and cords in a kitchen drawer accessible to the children. All of these cords were capable of creating a loop around a child’s neck. 110-300-0165(2)(d) Licensee had several plastic zip lock bags accessible to the children in a kitchen drawer. 110-300-0165(2)(f) Licensee had no stickers or art work at child’s eye level on the glass door. 110-300-0165(3)(a) Licensee had several boards stacked up in the outdoor place area that had splintered edges, licensee had protruding rusty bolts and nails located in two different window wells on the side of the house. Licensee had metal ramps that were located in the backyard by shed accessible to the children with sharp edges and large pair of adult sizzors in the kitchen drawer accessible. Licensee had a mirror and a glass toped scale in the bathroom that need to be removed from the licensed area or have plastic non-shatter proof placed over the top of each item. 110-300-0165(3)(e) Licensee had an uneven walkway rug that posed a tripping hazard in the living room. 110-300-0165(3)(f) licensee a large shelf in front of fire place, a kitchen set in the living room, and a glass round table accessible and not anchored in the living room posing a tipping hazard.
